International Affairs Division Announcement

IAD for International Students enrolled as Academic Year 2021 must complete both steps in order to fulfill health insurance requirements.

 

Undergraduate Student and Exchange Student

1. Student Report and Enrollment (send to Faculty)

1. Student Report Form (IR 9)
2. Copy of Passport Book
3. Copy of Transcript
4. 1 or 2 inch photo
5. Copy of health insurance contract or receipt along with summary of health insurance plan

Accident insurance will not be accepted as a replacement for health insurance.

 

2. Health insurance online report

After you have received your student ID card (about 2 weeks), please fill out the form at https://iad.kku.ac.th > INT’L STUDENT SERVICE > Health Insurance Report Form (E Form). The IAD will then submit the information to the Registrar Office once completed.

 

Postgraduate Student

1. Student Report and Enrollment (send to Graduate School)

1. Enrollment Form
2. Copy of Passport Book
3. Copy of Transcript
4. 1 or 2 inch photo
5. Copy of health insurance contract or receipt along with summary of health insurance plan

Accident insurance will not be accepted as a replacement for health insurance.

 

2. Health insurance online report
